We’re thrilled to have you join the Plum family. To ensure a smooth and seamless transition, we’ve made it easy to transfer your records from Compass Pediatrics into our system.
To get started, simply click the link below and complete the “Start Patient Transfer” form.
Once your form is submitted, our team will begin the process of transitioning your account and will reach out to secure your authorization for the transfer of prior medical records. Starting June 2, 2025 all transferred Compass Pediatrics patients will start having their appointments at Plum.
We’re here to make this transition as smooth as possible—and we’re honored to continue caring for your family.
